Dialogues about Supportive Care With Community Health Workers (CHW)

Diálogos sobre Atención de Apoyo con Promotores/as de Salud

Community health workers (CHW) and Promotores/as de Salud are joined by chaplains, psychologists, and physicians to discuss psychological, spiritual, and socio-behavioral components of patient care and how supportive care serves patients with life limiting illnesses.  Each conversation is a partnership between a CHW and a health care professional. Some topics are presented in English and other conversations are in Spanish. These conversations are intended for educational purposes as resource for CHW educators and individuals. Dialogues range from 25-50 minutes.

English-language topics include:

  • Conversations with Minority Communities about Palliative and End of Life Care:  A Community Health Worker and Chaplain’s Role and Value
  • Supporting Patients with Patient-Centered Communication Skills
  • Attending to Clients and Their World: A Closer Look at the CHW Relationship

Spanish-language topics include:
